Our goal is to have every patient leave our office knowing they received the services they need to maintain a healthy mouth.

We are currently accepting new patients. Please call our office to schedule an appointment. Once you make an appointment at our office, that time will be reserved for you. If, for some reason, you are unable to keep your scheduled appointment time, call us as soon as possible to reschedule to avoid missed appointment fees.

  • What to Bring to your First Appointment:

To provide you with prompt dental treatment, please be sure to bring the following when you visit our practice for your initial visit:

 

  • Insurance Information/Card
  • Photo Identification
  • Any medical records pertaining to your visit

 

Please arrive 15 minutes before your scheduled appointment time to fill out necessary paperwork. If you would like to complete your paperwork prior to your appointment, you may print and fill out the appropriate forms. 

  • Insurance:

Insurance coverage varies from policy to policy, and it’s your responsibility to understand what your insurance plan covers. Questions regarding your individual plan should be directed to your human resources department or insurance provider.

 

As a service to our patients we will file insurance claims with most traditional plans and are a preferred Delta Dental provider. We make every effort to maximize your dental benefits, and we will provide you with a complete estimate before any treatment is done. Insurance co-payments are to be paid at the time of service. You may need to have pre-authorization from your insurance company prior to starting treatment and we will be happy to submit that for you.

  • Payment:

Payment is expected at the time services are rendered, unless other arrangements have been made. This includes any co-payments or deductibles (amount not covered by your insurance). For your convenience, our office accepts cash, personal checks, Visa, MasterCard & CareCredit.
